TOLEDO, OH (Toledo News Now) - The City of Toledo has made the report on the 2014 fire that killed two firefighters public.
Toledo News Now obtained a copy of the report Thursday.
Firefighters Stephen Machinski and Jamie Dickman were killed battling the fire at 528 Magnolia. The fire was investigated as an arson and building owner Ray Abou-Arab is awaiting trial on murder charges.
Among the findings, the internal report says the very important safety officer was the last position assigned to the fire - more than 16 minutes elapsed since the initial alarm. The report says command must fill the duties and responsibilities of safety until command assigns safety to another officer.
That mirrors findings of the NIOSH report, a federal report on the deadly fire.
Mayor Paula Hicks-Hudson spoke on the internal report at at a news conference Friday at 10 a.m. She said that while many issues ultimately lead to the deaths of the two firefighters, "the initial contribution was the arson itself."
Toledo Fire Department Chief Luis Santiago also addressed report, saying "This was a culmination of events that really, in it's totality, caused what happened that day."
